2023年全国杰出华文教师奖提名表格 Nomination Form For Distinguished Chinese Language Teachers Awards

条规：

• 所有本地教育部属下小学、中学、高中及初院的在职华文教师都可被提名（曾获本奖项以及“全国模范华文教师奖”者除外）
• 被提名教师应至少有五年的教学经验。
• 曾获提名的华文教师可间隔至少一年后再被提名，例如在2021年被提名的教师，可在2023年重获提名。
• 入围终选的教师须获得校长书面认可支持，并于2023年7月至8月间，接受评审的课堂评鉴与面试。
• 得奖者名单预计将于2023年9月份公布。
• 主办单位与评审拥有决定最终得奖者人数的权利。

奖励：杰出华文教师各可获得1500元现金及奖座一个。

Terms & Conditions:

• All Chinese language teachers in MOE primary, secondary, pre-universities, junior colleges and IP schools are eligible (except for past winners of this award)
• Nominated teachers should have at least five years of experience teaching in these MOE schools. 
• Previously nominated teachers can be nominated again after at least a lapse of one year. For example, teachers nominated in 2021 or earlier could be re-nominated in 2023. 
• Classroom observations and interview with judges for shortlisted nominees will be conducted in July/August 2023. Endorsement from respective schools‘ principals is mandatory.  
• Winners will be announced in September 2023 (subject to change).
• Organisers and judges reserve the right to decide on the number of winners awarded.

Prizes: All winners will receive $1500 cash and a trophy each
